Registration forms for Tyngsboro Youth Lacrosse
To register for the 2008 TYLA season, please complete the US Lacrosse registration and then the TYLA Registration, you will need your US Lacrosse Number to complete the TYLA Registration .
1. TYLA Registration Form and Waiver. This is required of all participants. It also includes the US Lacrosse Code of Conduct agreement. Please make sure that both pages of either version (.doc or .pdf) are filled out completely and signed. In particular, we need to make sure that insurance information is correct. Please remember to include 2 checks. The first one is for the registration fee. The second (for $35) is for the jersey deposit, which will be returned at the end of the year.

2. US Lacrosse Association. Players are required to complete the US Lacrosse Application. Please go to www.uslacrosse.org and complete the online application (follow the link below and select "Apply OnLine"). Please include your US Lacrosse number on your TYLA form.
Please note that registrations will be accepted on a first come first serve basis. Registrations for each team will be capped at 20 per team, with the remainders placed on a waiting list. Registrations can also be mailed before the registration night to Tyngsboro Youth Lacrosse, PO Box 723, Tyngsboro, MA 01879.
